Vaginal slings or urinary incontinence slings are medical devices placed under the urethra through a small incision to treat incontinence symptoms. Female stress urinary incontinence (SUI) can be classified in two ways: intrinsic sphincter deficiency (ISD) and urethral hypermobility. Slings are commonly used in women who have urethral hypermobility. A sling will stop the hypermobility, but the patient may still have stress urinary incontinence due to an ISD. ISD occurs when the urethral sphincter is unable to generate enough resting pressure to prevent urine from seeping out. Urethral hypermobility, also known as Type II SUI, is due to weakened anatomic urethral support, often caused by childbearing. Vaginal slings directly compete with bulking agents; however, slings are considered the leading technology for treating SUI. Female incontinence sling procedures include the installation of transobturator tape sling (TOT), tension-free vaginal tape (TVT), also known as retropubic mid-urethral slings, and single-incision, also known as mini-slings, for SUI. These procedures can also be used to address pelvic organ prolapse (POP), which is not the main focus of this report. TOTs were introduced into the U.S. in 2002 and are implanted using tension-free surgery procedures. These procedures are considered less invasive than those needed to install TVTs as a needle is not blindly inserted through the retropubic space when installing the mesh tape. Procedures to implant single-incision slings can carry fewer risks of complications than TOTs and TVTs. A single, small incision is made and mesh tape is inserted through the mid-urethra. The vaginal slings included in the report are those that are implanted by traditional as well as single-incision implantation.
