There is growing evidence that they could also be promising therapeutic targets in a wide range of conditions, including cerebral and cardiac ischaemic diseases, sleep disorders, immune and inflammatory disorders and cancer. After more than three decades of medicinal chemistry research, a considerable number of selective agonists and antagonists of adenosine receptors have been discovered, and some have been clinically evaluated, although none has yet received regulatory approval. There are four known subtypes of adenosine receptors (ARs) – referred to as A1, A2A, A2B and A3 – each of which has a unique pharmacological profile, tissue distribution and effector coupling. All four subtypes are members of the superfamily of G-protein-coupled receptors (GPCRs), and are most closely related to the receptors for biogenic amines. Among the human ARs, the most similar are the A1 and A3 ARs (49% sequence similarity) and the A2A and A2B ARs (59% similarity). Selective antagonists have entered clinical trials for the treatment of Parkinson’s disease and congestive heart failure. Both in the case of diseases such as stroke, where there is an unmet medical need, and for diseases that already have pharmacological intervention options, the introduction of adenosine-based drug therapy will provide novel mechanisms for therapy.
