Pethidine or meperidine is sold under the brand name of Demerol and it is a synthetic opioid pain medication of phenylpiperidine class. Meperidine is indicated for the treatment of moderate to severe pain. It can be administered as hydrochloride salt in tablets, as a syrup or by intramuscular, subcutaneous or intravenous injection. Due to its toxic metabolite, norpethidine, it is more toxic than other opioid in long term usage. The metabolite of drug has serotonergic effect that could contribute to serotonin syndrome. Meperidine is a widely used opioid in labor and delivery. Meperidine is the preferred painkiller for diverticulitis because it decreases the intestinal intraluminal pressure. This drug is also used for the management of shivering during therapeutic hypothermia, as it provides the greatest reduction in the shivering threshold. It is probable that meperidine can also interact with a number of other medications including muscle relaxants, benzodiazepines, and ethanol. The adverse effects of meperidine administration are primarily those of the opioids as a class: nausea, vomiting, dizziness, diaphoresis, urinary retention, and constipation.
