Lutetium-77 is a radioactive lanthanide isotope with a half-life of about 6.65 days, and releases energy mainly through beta decay. It has important applications in the medical field, especially in cancer treatment. For example, Lu-177-labeled prostate-specific membrane antigen (PSMA) radioligand therapy (such as Lu-177-PSMA-617) is used to treat metastatic castration-resistant prostate cancer (mCRPC). This therapy combines radioisotopes with PSMA-targeted drugs to precisely deliver radioactive substances to prostate cancer cells, thereby achieving precise treatment of tumors.
Lu-177-PSMA-617 has shown significant efficacy in clinical trials. For example, in a phase III clinical trial, Lu-177-PSMA-617 significantly prolonged patients' progression-free survival (PFS) and overall survival (OS), and reduced the risk of disease progression or death. In addition, Lu-177-PSMA-617 also showed better survival and progression-free survival than traditional chemotherapy drugs (such as cabazitaxel) in actual applications.
In addition to prostate cancer treatment, Lu-177 is also used to treat other cancers, such as neuroendocrine tumors (NETs). For example, Lu-177-DOTATATE (Lutathera) is a radiopharmaceutical used to treat gastroenteropancreatic neuroendocrine tumors, which has been approved by the US FDA and widely used in clinical practice.
The production of Lu-177 is mainly achieved through nuclear reactors or accelerators. China has also made significant progress in the production of Lu-177 and is able to meet the domestic demand for this radioisotope.
