Knee denervation is a surgical treatment for knee pain and other related diseases. Its core principle is to cut off or destroy the sensory nerve branches of the knee joint, thereby blocking the transmission of pain signals, and ultimately achieving the purpose of relieving or eliminating pain. The surgery is mainly for patients who have been suffering from knee pain and have not responded to conservative treatment, aiming to improve their quality of life and reduce the hindrance of pain to daily activities.

The pain sensation of the knee joint originates from the perception of the nerve endings around the joint, and then the signal is transmitted to the central nervous system. When the knee joint has lesions, such as osteoarthritis, rheumatoid arthritis, etc., these nerves will be stimulated and produce pain signals. Knee denervation cuts off or ablates the nerves to prevent the transmission of pain signals, so that the brain cannot receive pain information from the knee joint, thereby achieving pain relief. At the same time, denervation may affect the local nerve regulation function of the joint, and improve the inflammatory state and function of the joint to a certain extent.

In terms of surgical methods, there are mainly radiofrequency ablation denervation and surgical denervation. Radiofrequency ablation denervation is more commonly used. Under the guidance of imaging equipment, the doctor accurately inserts the radiofrequency electrode needle into the nerve branches around the knee joint, and uses the heat generated by the radiofrequency current to cause coagulative necrosis of the nerve tissue and destroy the nerve conduction function. This method has accurate positioning, less trauma, and fast recovery. Surgical nerve cutting directly cuts off specific nerve branches around the knee joint under direct vision or with the help of arthroscopy. This method can directly remove the pain conduction pathway, but the trauma is relatively large, the postoperative recovery time is long, and there is also a risk of complications such as local hypoesthesia and muscle atrophy.

Knee denervation has been used to a certain extent in clinical practice. It provides an effective treatment option for patients with specific types of knee pain such as middle and late-stage knee osteoarthritis when drugs and physical therapy are not effective. Some patients have significantly reduced pain after surgery, improved joint function, and enhanced ability to perform daily activities such as walking and going up and down stairs. However, this surgery is not suitable for all patients with knee pain, and there are individual differences in efficacy.

Looking ahead, with the development of technology, knee denervation is expected to be deeply integrated with the concept of precision medicine. With the help of more advanced imaging and nerve localization methods, pain-causing nerve branches can be more accurately identified and treated, improving the efficacy and safety of surgery.
