Tonic Spinal Cord Stimulators (SCS) are implantable medical devices designed to manage chronic pain by delivering continuous (tonic) electrical pulses to the spinal cord. These pulses interfere with the transmission of pain signals to the brain, offering relief primarily for neuropathic pain conditions such as failed back surgery syndrome, complex regional pain syndrome, and peripheral neuropathy. The tonic stimulation mode maintains a consistent frequency and amplitude, typically in the range of 40–60 Hz, which produces a tingling sensation (paresthesia) that masks pain perception. While newer stimulation paradigms like burst or high-frequency stimulation have emerged, tonic stimulation remains a widely used and foundational approach in spinal cord neuromodulation therapy.
Tonic Spinal Cord Stimulators (SCS) deliver continuous low-frequency electrical pulses, typically ranging from 40 to 60 Hz, with adjustable pulse widths of 50 to 500 microseconds and amplitudes between 0.1 and 10.0 mA to mask chronic pain sensations. These systems feature single or dual leads with 4 to 16 electrodes, and are powered by either rechargeable or non-rechargeable lithium-ion batteries, offering 3 to over 10 years of device longevity. The implantable pulse generators (IPGs) are compact, generally 45–55 mm in diameter and 8–10 mm thick, and are programmed via clinician-operated tablets or handheld devices. Most modern units are MRI-conditional and may also support additional waveform options, although they are primarily designed for tonic stimulation. IPGs are typically implanted in the lower back or buttock region, and rechargeable models require periodic recharging, typically every few days to weeks.
