The pathogenesis of BPDCN is not fully understood, but it is believed to arise from plasmacytoid dendritic cells. These cells are a subset of dendritic cells that play a crucial role in innate and adaptive immune responses. The neoplasm demonstrates frequent deletion of tumor suppressor genes such as RB1, CDKN1B, CDKN2A, and TP53, which likely contribute to its aggressive behavior.
Clinically, BPDCN often presents with asymptomatic, solitary, or multiple skin lesions, which can manifest as nodules, plaques, or bruiselike lesions. These lesions may vary in size and can be accompanied by erythema, hyperpigmentation, purpura, or ulceration.
