There are no approved drugs for VaD, although companies currently marketing AD products are likely to launch their drugs in 2003. Further drug launches will contribute to the rapid growth of this attractive market, which is predicted to be worth $1.9bn in 2008. DLB is the third most prevalent dementia, although it has only recently become recognized as a clinical entity. Consequently, DLB is associated with very low diagnosis rates. However, physicians diagnosing DLB commonly prescribe AChEIs off label. Novartis is in the process of gaining approval for Exelon for a 2002 launch and will initially dominate the market, which is expected to be worth $490m in 2008. Although antiviral combinations can prevent ADC, and sometimes reverse it, there remains a strong unmet need for cognition enhancing drugs. The most promising candidates thus far are NMDA antagonists and MAO inhibitors. Estimated to be worth $119m in 2008, ADC is clearly a niche market ideal for small pharmaceutical companies marketing antidementia drugs. BPSD affect over 80% of dementia patients, and are widely regarded as the most distressing symptoms for both patient and caregiver. The greatest opportunity for pharmaceutical companies lies in the treatment of psychosis and aggression with atypical neuroleptics. For example, sales of drugs in this class are expected to reach $714m in 2008 for the treatment of delusions in dementia patients.
