CHAPTER 2 PIPELINE OVERVIEW AND DYNAMICS

Pipeline overview

Drugs in the upper GI disorder pipeline fall into a wide variety of classes, but proton pump inhibitors remain the focus of research

Key companies involved in the upper GI disorder pipeline

AstraZeneca: a dominant force in the GI disorder market, but for how much longer?

AstraZeneca developed and marketed Losec/Prilosec (omeprazole), the first proton pump inhibitor available for the treatment of GERD

Nexium (esomeprazole) has been a driver of growth for AstraZeneca

Nexium is available in both oral and intravenous formulations

AstraZeneca will move its focus away from core research areas including gastroenterology possibly driven by frustration at discontinued projects

Challenges to the patents covering Nexium

AstraZeneca has three products in the pipeline for upper GI disorders

TAP Pharmaceuticals: in-licensing the central strategy to success

Prevacid (lansoprazole) is the sole product in TAP Pharmaceuticals' GI franchise

Prevacid patent expiry in the US will contribute to slowing growth at TAP Pharmaceuticals

TAP Pharmaceuticals looking to follow on from Prevacid with dexlansoprazole and ilaprazole

Key R&D company strategies

Isomers of PPIs

Combination therapies

Indication expansion

New PPI formulations

Definition of current comparator therapy

Dexlansoprazole

Drug overview

Clinical trial data

Clinical trial 1

Patient potential

Dexlansoprazole will be beneficial in all GERD and PUD patients

TAP Pharmaceuticals will be looking to switch patients from Prevacid (lansoprazole) to dexlansoprazole (TAK-390MR)

Marketing factors

TAP Pharmaceuticals was working with Flamel Technologies to develop a controlled-release formulation of lansoprazole; however, TAP decided to pursue dexlansoprazole

TAP Pharmaceuticals has vast marketing experience in the area of upper GI disorders, which will benefit dexlansoprazole

Satisfaction of unmet needs

Forecasts to 2016

S-tenatoprazole-Na

Drug Overview

Clinical trial data

Clinical trial 1

Clinical trial 2

Clinical trial 3

Patient potential

Small scale clinical data from the Japanese population demonstrate that racemic tenatoprazole is comparable to omeprazole in the cure rate of peptic ulcers

Early clinical studies suggest a faster onset of action and longer duration of action for tenatoprazole compared with esomeprazole

Marketing factors

Sidem Pharma has taken over the development of tenatoprazole from Mitsubishi-Tokyo Pharmaceuticals

Tenatoprazole was undergoing Phase II clinical trial, but these have been suspended

Sidem Pharma and Steba Laboratories will need to acquire an experienced marketing partner if they are to take on the marketing power of AstraZeneca

Satisfaction of unmet needs

Forecasts to 2016

AGI-010

Drug overview

Clinical trial data

Patient potential

AGI Therapeutics will be looking to position AGI-010 as a treatment for nocturnal acid breakthrough in the upper GI market

Marketing factors

AGI Therapeutics and Axcan Pharma have signed a co-development agreement for AGI-010

Axcan is an international company with a number of products in the GI market including a 3-in-1 therapy for the eradication of H. pylori and treatment of PUD

Satisfaction of unmet needs

Forecasts

OX 17

Drug overview

Clinical trial data

Patient potential

OX 17's rapid onset of action and long duration of action will make this drug desirable; however, the perception is that co-administration of an H2 blocker with a PPI is not feasible

Marketing factors

Orexo's lack of experience will result in the company seeking a marketing partner for OX 17

Orexo may position OX 17 as an over-the-counter drug in the US and EU

Satisfaction of unmet needs

Forecasts

Ilaprazole

Drug overview

Clinical trial data

Preclinical data

Phase II clinical trial

Planned US Phase II study and reported Chinese Phase III completion

Patient potential

Early Phase I studies show that ilaprazole is superior to omeprazole at reducing acid output in patients with GERD

Marketing factors

Il Yang does not have a strong presence outside of South Korea

TAP Pharmaceuticals will be responsible for the worldwide marketing of ilaprazole

Satisfaction of unmet needs

Forecast

Other novel late-stage acid suppression therapies

Revanex (revaprazan)

Soraprazan

Soraprazan progressing slowly through clinical trials and unlikely to reach market before Protonix patent expiry

Dexloxiglumide

CCK antagonists will not have great potential in GERD because of low efficacy compared to PPIs

Z-360

Other acid suppression therapy under investigation for additional upper GI approvals

Omeprazon (omeprazole)

Famotidine (YM1170)

Protonix (pantoprazole)

Late-stage development compounds recently suspended or discontinued

CS-526

CS-526 was being co-developed by Daiichi Sankyo and Novartis

The discontinuation of CS-526 means that Novartis does not have a presence in the upper GI disorder market

AZD0865

AZD0865 is a potassium-competitive acid blocker that was being investigated for GERD

AZD0865 was terminated in Phase II development because it showed signals of hepatotoxicity and failed to show clinical superiority over esomeprazole

Clinical Trial 1

Clinical Trial 2

Patient potential

Axcan has developed the first 3-in-1 combined tablet for the treatment of H. pylori eradication; however, availability of generics will limit its potential

Clinical trial data demonstrate efficacy in patients with clarithromycin- and metronidazole-resistant H. pylori

Pylera will have a reduced patient potential as it has not demonstrated improved compliance over comparator drugs

Marketing factors

Axcan has previous experience in the GI market which will be advantageous to Pylera

Satisfaction of unmet needs

Abstract

Introduction

Gastroesophageal reflux disease (GERD), functional dyspepsia (FD) and peptic ulcer disease (PUD) represent a group of common gastrointestinal (GI) conditions, which can be referred to collectively as upper GI disorders. Gastric acid suppression remains the cornerstone of therapy in upper GI disorders, with particular focus on reducing symptoms and eradicating H. pylori, if present.

Scope

Analysis of patient potential, unmet needs and clinical trials in upper GI disorders. Overview of drugs in late- and early-stage clinical development; with analysis of key companies involved in the R&D pipeline. Detailed profiles of key compounds in development for use in upper GI disorders, with forecasts of drug revenues to 2016. Discussion of R&D company strategies used in upper GI disorder market, focusing on AstraZeneca and TAP Pharmaceuticals.

Highlights

The upper GI disorder pipeline is small, with only 33 products, and only 3 drugs in Phase III. In a market that is dominated by PPIs, which are becoming increasingly genericized, and with over-the-counter medicines available, pharmaceutical companies appear to be less than willing to enter this competitive market. A key company R&D strategy is the development of specific enantiomers of PPIs. This extends patent life and can result superiority to the racemate mixture. TAP Pharmaceuticals has for dexlansoprazole, and Sidem Pharma and Steba Laboratories are co-developing S-tenatoprazole for GERD. Both are expected to quickly achieve blockbuster status. The inclusion of Nexium (esomeprazole) as a comparator in clinical trials is essential to ensure market success for pipeline products. Companies will also benefit by addressing the unmet needs such as drugs for the PPI unresponsive population. This niche patient subgroup is difficult to treat and is currently underserved.

Reasons to Purchase

Understand unmet needs in the upper GI disorder market based on key opinion leader comment. Benchmark key late-stage pipeline products against the current market leader. Assess the sales forecasts of key late-stage pipeline drugs and examine their clinical and commercial potential.
