Bridging Adult Day Services and Hospice/Palliative Care: Enhancing End-of-Life Care Through Collaboration

The number of persons age 65 and older is estimated to grow by 80 percent creating an even more critical need for direct services for individuals approaching the end of their lives. A portion of the program will delve into the results of an Easter Seals initiative "Enhancing Community-Based Options for End-of-Life Care." Easter Seals’ project is creating tools and training resources to help adult day services personnel and hospice/palliative care personnel across the country better understand each other's roles and integrate clinical practices programs and policies. This program will also address how policy is being approached at the state and national level to reduce barriers and promote collaboration between these two vital communities of care. No one should have to approach death without pain management compassion and support. This initiative will help begin to make a real difference for many families during such a difficult time.
