This research led to the development of Models of CareTM reports.Models of CareTM reports are targeted for nurses, physicians, and other health care professionals and administrators in managerial or leadership positions. Each report analyzes the trends in health care delivery programs for specific clinical conditions and describes seventeen to twenty case studies in detail. For each issue, ten case studies come from hospitals, health systems, managed care plans, and other providers, and the other seven to ten come from employers, coalitions, government agencies, vendors, and other support organizations. The case studies are based on indepth interviews with health care professionals who are involved with the development and implementation of the programs. Case studies include information on the steps of care of the program, how physicians are involved, administrative needs, keys to success, barriers, measured outcomes, and words of advice.
